As an adult patient, you are responsible for your healthcare. You have to stick up for yourself, raise your hand and ask questions when you don't understand. It takes some practice, but preparing for transition was your practice. You're now ready to take on the responsibility for your own care. A couple of things to remember:

  • Make sure you put your health in first place
  • See your new doctor regularly
  • Being an adult means you prevent problems before they happen
  • Don't wait until you have a problem to seek medical care

Tips for success

You already know that taking good care of your health is important. Pain is the most common complication of SCD, and it's the number one reason that people with SCD go to the emergency room or hospital.

Don't forget to:

  • Drink plenty of fluids to avoid becoming dehydrated and increasing your risk of a pain crisis
  • Get plenty of sleep
  • Eat a well-balanced diet
  • Exercise but don't overdo it
  • Take your medicine
  • Stay up to date on vaccines

Shared decision-making

Every decision about your care — your treatment, medical tests, and health issues — comes with pluses and minuses. This is why shared decision-making is important. It's a partnership between you and your doctor. The idea is to incorporate your values and what you want to do along with your best available options. This way, you can know and can discuss your care, understand why it may be necessary, build a trusting relationship with your doctor, and feel good about participating in your own healthcare decision-making.
